Usually around 40 unless there are additional issues

If you are otherwise healthy, an annual rectal prostate exam by your doctor should start about age 40. Tumors are very rare in younger people. Most enlarged prostates are benign but do enlarge as a person gets older and may lead to difficulty in urinating. Rarely, a teenager may contract gonorrhea which could invade the prostate and produce pain and difficulty in peeing. If so, a prostate exam, massage, and antibiotics may be very important to prevent further spread of this sexually transmitted disease.

If there is no history of prostate cancer in your family, I would recommend your first exam at the age of 50. Then at least PSA tests every three years. If there is any significant rise in your PSA, your Dr. will recommend further exams.After 60 I would recommend a full exam every three years and a simple PSA test every other year. I am a prostate cancer survivor now in my 5Th year.

Most doctors recommend beginning exams at age 40, but if there is a high risk of cancer (i.e. runs in the family) he may wish to begin these exams at age 30 instead.
